When making an appointment please have your insurance/method of payment available. Also, if you need special assistance at your appointment, such as an interpreter, please note this when making your appointment.

When preparing for your appointment,
please bring a copy of:

  • Health insurance cards

  • Photo ID

  • Your current medications, including nutritional supplements

  • Proof of income documents
